Leaving our practice - Transferring your medical records

When transferring your medical records to another GP/medical practice, please be aware that you will be transferring your future care to your new doctor and will no longer be a patient of this practice. We will no longer be able to provide you with scripts, referrals or appointments.

This does not apply for information sent to specialists, allied health etc.

Your new GP will have forms available that will request your records. Please contact your new provider for more information.
